For those who are about to be discharged from hospital – after recovering from surgery, a fall or illness – but are not yet confident enough to return home, Gwen Walford House provides purpose-built residential accommodation with 24 hour nursing care; the ideal step-down setting to heal and quickly regain your strength, mobility and independence.
Our team plan individual care for each resident, developed in conjunction with the hospital discharge department and including:
The comprehensive nursing care at Gwen Walford House is of the highest standard, with all our care supervised by Registered General Nurses.
Since frail elderly patients are especially at risk of complications, relapses or accidents after a stay in hospital, our staff are also supported by GPs, therapists, dieticians, social workers and discharge co-ordinators.
We can also provide a range of rehabilitation and therapy services on-site.
At Gwen Walford House, we really do aim to get to know our residents and their families as we believe our exceptional care can help improve your condition, so you can return home or to your daily routine sooner and better prepared.
