If our residents are to feel safe and comfortable in an environment that is familiar to them so they develop a sense of belonging to the Hampton Grange community, the care we provide needs to be unique to each individual and based on their own personal likes, dislikes and habits.
With the help of family, friends, Social Services and Healthcare Workers, we spend time getting to know every new resident, so we can draw up an individual care plan to address their own specific medical, physical, intellectual and social needs.
This care plan is kept by the patient’s bed. It is not a secret record – both patients and their relations are encouraged to read it and add suggestions, in accordance with the Data Protection Act 1998.
