Hampton Grange and
Gwen Walford are known for delivering high-quality convalescent, residential, palliative, dementia, and nursing care. We work closely with other professionals, including St Michael's Hospice, GPs, and Mental Health professionals, to take a multi-disciplinary and collaborative approach to our care. Our highly trained professionals work internally and externally across departments to provide a holistic care experience tailored to each resident.

As Clinical Lead, Oana plays a pivotal role in supporting our nursing team, leading in areas such as care plan development, medication management, and wound care. · A testament to Oana's exceptional skill and dedication is the remarkable transformation of one of our residents. Arriving for end-of-life care with a complex and severe wound, the resident faced a challenging prognosis. Under Oana's meticulous care and leadership, her condition improved dramatically. Within three months, the wound healed significantly, pain management was reduced, and the resident regained her independence. Today, she enjoys socialising in our communal areas, engaging in activities, and cherishing visits from family and friends.
· One resident arrived from another home for end-of-life care. Recognising this need early, we involved a palliative nurse to help the family understand the situation and plan the best course of action. We also arranged for a vicar to visit, respecting the resident's religious needs.
· Another resident arrived with a perforated bowel and was given end-of-life care. Thanks to our team's care plan and consistent attention, she recovered and was eventually able to move to a residential home, no longer needing nursing support.
· DL was admitted to Gwen Walford House for end-of-life care in December 2021. Under our care, DL's condition improved significantly. We supported him in obtaining an electric wheelchair, which he now uses, allowing him to regain his independence. He even enjoys a bit of gardening, reflecting the positive impact of the care he received.
